Home Care Registered Nurse (RN)

$10K sign on bonus

Provides skilled nursing care to home-based patients in accordance with physician orders and organizational policies. Performs comprehensive assessments, develops and manages individualized care plans, and collaborates with interdisciplinary teams to optimize patient outcomes. Oversees and supports LPNs and Home Health Aides. Ensures compliance with regulatory standards, promotes patient satisfaction, and contributes to quality improvement initiatives.

Requirements

• Current RN license: BSN preferred
• Minimum 1 year of nursing experience (required in CA)
• Home healthcare experience preferred
• Valid driver’s license and reliable transportation
• Strong computer skills and commitment to ethical, mission-driven care

Saint Agnes Home Health and Hospice is a member of Trinity Health At Home, a national home care, palliative care and hospice organization serving communities in twelve states. We are a trusted provider of home care and hospice in the sacred place that people call home. A Faith-based, non-profit organization, we serve patients and their loved ones with home care (skilled nursing, physical/occupational and speech therapy and medical social work), hospice and bereavement services. Mental and Physical Requirements:

• Frequently lift, stands, and walks (35% of time)
• Occasionally bend, stoop, and twist (10% of time).
• Occasionally position and lift patients (10% of time).
• Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time).
• Must be able to adapt to frequently changing work parameters.
• Ability to independently lift up to 35 pounds.
• Must be able to travel to various TH/THHS worksites or various work locations as required.
